Group 1 Application Forms
All of the forms listed below are required for the Group 1
Application Package, (Companies seeking to conduct business with
Arizona gaming facilities for the first time). Open each form and print it. All listed
forms must be completed and submitted to the Department of Gaming to
start the certification process. A set of individual forms must be
printed and completed for each person designated as a key person
associated with the business.
